Background: Prediabetes represents an intermediate metabolic state between normoglycemia and diabetes mellitus, characterized by impaired fasting glucose, impaired glucose tolerance, or elevated HbA1c levels. Insulin resistance (IR) is a key underlying mechanism driving the transition from prediabetes to type 2 diabetes mellitus (T2DM). The triglyceride–glucose (TyG) index, derived from fasting triglyceride and glucose levels, has emerged as a simple, cost-effective surrogate marker of insulin resistance. However, limited data exist on its association with HbA1c among prediabetic individuals in the Indian population.
Aim: To evaluate the association between the triglyceride–glucose (TyG) index and glycated hemoglobin (HbA1c) levels in prediabetic and healthy individuals.
Materials and Methods: A hospital-based cross-sectional case–control study was conducted at RajaRajeswari Medical College, Bengaluru, that included 100 participants (50 prediabetics, 50 matched controls; aged 30–60 years). Prediabetes was diagnosed as per ADA criteria. FPG, TG, and HbA1c were measured by standard enzymatic and HPLC methods. TyG index = Ln [(Fasting Triglycerides×Fasting Plasma Glucose) / 2]. Data were analyzed using SPSS v25.0 with t-test/Mann–Whitney U test and Pearson's correlation.
Results: Both Groups were age and sex matched (p > 0.05). Prediabetics had significantly higher FPG, HbA1c, TyG index and TG (p < 0.001) compared to controls. There was significant but weak positive correlation between TyG index and HbA1c in prediabetics. (r = 0.323, p = 0.022), but no significant correlation between them (r = 0.135, p = 0.350) in healthy controls.
Conclusion: Prediabetics have high TyG index value and it correlates with HbA1c levels. This suggests TyG index may serve as a surrogate marker for insulin resistance, and monitor progression of dyslipidemia in prediabetes. Larger Longitudinal studies are further required to determine clinical decision points.
