Clinico-Observational Assessment of Amalki Rasayan on Total Iron Binding Capacity in Iron Deficiency Anemia

ABSTRACT

Anemia is categorized by low hemoglobin levels, depleted iron stores, and elevation of total iron-binding capacity (TIBC). Our study evaluated the efficacy and effect of Amalki Rasayan (Phyllanthus emblica Linn.) on iron metabolism in 50 patients with confirmed Iron Deficiency Anemia (IDA), in comparison to conventional oral iron drug. Treatment with Amalki Rasayan lead to noticeable reduction in TIBC from raised baseline values (412–518 µg/dL; mean ≈ 462 µg/dL) to near-normal post-treatment levels (325–397 µg/dL; mean ≈ 358.94 µg/dL), with individual reductions ranging from 41 to 171 µg/dL. Hemoglobin levels increased constantly from 7.5–9.2 g/dL to 10.1–16.9 g/dL, with expressively greater mean increase compared to the reference iron drug (2.63 ± 0.34 vs 1.76 ± 0.29 g/dL; p < 0.05). The TIBC decrease was also greater (−88 ± 11 vs −64 ± 10 µg/dL; p < 0.01). Constant study demonstrated monthly increase in serum iron levels (total increase between 45–51 µg/dL; p < 0.001), accompanied by significant enhancements in RBC count and normalization of mean corpuscular volume MCV (p < 0.001). The Friedman and Wilcoxon studies were confirmed by highly significant changes among all hematological parameters (p < 0.001). Spearman's correlation analysis shows strong negative correlations of TIBC with Hb (ρ = −0.72) and ferritin (ρ = −0.75). Our study shows a strong positive correlation between hemoglobin and ferritin (ρ = 0.68); it indicates coordinated restoration of iron homeostasis. Prominently, Amalki Rasayan was well acknowledged with negligible adverse effects in comparison to gastrointestinal intolerance frequently observed with oral iron drugs. Our findings provide effective biochemical and statistical evidence in support of the effect of Amalki Rasayan and endorse its potential as an effective, safe, and integrative therapeutic alternative for the management of iron deficiency anemia.
